Registered Nurses and Licensed Social Workers have experience in coordinating care for seniors, including home health care. Because they are local, they are able to better help families find the care their loved ones requires. Some insurance plans will offer this service as a benefit to the insured. This service is both helpful to the patient as well as their family as the coordinators will be very knowledgeable in what care is needed and what is available in the area. They will be available to guide you in your care.
Some insurance companies will require the use of a certain coordinator while others will allow the family to use the coordinator of their choice.
Some tasks that coordinators will be able to assist with in your care:
- Educate patients about the role they will play in their care as well as the role of the coordinator themselves.
- Develop a care plan that will outline what responsibilities the family will have along with what will be provided by different providers within the community.
- Provide educational material related to treatments, procedures, medication and care requirements that the patient will be receiving.
- Coordinate with providers to make sure that all needs are met in a timely fashion. Coordinators will also coordinate that supplies are delivered and set up as needed as well.
- Monitor delivery of care provided by outside providers.
- Help solve any problems that may arise to prevent stress from being placed on the family.
