Each person is different. This means that in order to get the right nursing home care plan, you should develop a care plan that fits your needs. It is up to you to make your goals, wishes and health care requirements known to the staff of your nursing home.
Health Assessment
The first day you are admitted to a nursing home, you will be given a heath assessment. This assessment must be completed in fourteen days. During this assessment, the nursing home staff will determine your health condition. Every 90 days, they will compare your current condition to your previous assessment. The home must then submit this info to the federal government.
Things to Include on your Health Care Plan
Here are some things that you might want to include on your health care plan:
- What services do you need? Are there any special needs the staff might not know about?
- Make sure there is specialized staff that might need to address your health care needs.
- Add a schedule of your care. Include daily, weekly and monthly milestones.
- Consider any special dietary considerations
- Include health goals and any other personal goals you might have.
- Future plans, including a return to more independent living if at all possible.
