As people age it can become more difficult for them to remain in their home. This can be due to any number of factors including lowering of cognitive functioning, health issues or other physical limitations. Unfortunately for many seniors, their home is a much better place for them to age than an assisted care facility. Typically this is due to financial situation, but it can also be for other reasons.

The Need for Care

When seniors need care in their home it often falls on family members to bear the burden of the care. The problem that exists today is that many family members are geographically distant from their elder loved ones. The same can apply for necessary services that are not available in more remote locations. This leaves a gap in the care that can be given to these seniors, leaving them at risk for injury or even death.

Geriatric Care Management

As this segment of the population has grown, the need for someone to fill the gap has been recognized. That is where Geriatric Care Management can help. GCM’s are there to help seniors find the necessary services for them to remain in their homes.

GCMs work with family members, seniors and other care providers to fill the gap that is left by a family who wants to help but can’t because of either distance or time. GCMs will help to plan care visits, help seniors get to needed medical appointments and generally watch out for the seniors in their care.

Currently Medicare and Medicaid will not pay for these services, but the cost of a GCM is often much less than that of an assisted care facility.
